Access to Your Records

Visit http://my.sjsu.edu to access your records online after you register. If you cannot remember your user ID and password, please email the CMS Help Desk cmshelp@sjsu.edu.

Provide at least two of the following items for verification:

  1. SJSU student ID
  2. Date of birth
  3. Mailing address

Administrative Fee


The university has the authority to charge an administrative fee of $20 for failure to meet a due date. Payments received after the due date are subject to this charge.

Spring 2013 Petition for Course Drop

INSTRUCTIONS

(Use the Petition for Withdrawal if you are dropping all courses in the semester)
NOTE: Although this petition displays the current semester’s date, it should also be used for course drops from previous semesters. Your petition will not be processed if you are not using the current form. Please visit www.sjsu.edu/aars/forms to be certain that you are using the most recent version of this petition.
Course Drops may be approved under several categories. These are based on (1) the date when this petition is submitted, and (2) the reason(s) for the request. Petitions may be submitted in person (to Academic Advising and Retention Services, Student Services Center) or by fax (408.924.2526).

There are three time periods relating to when this petition is submitted:

A. After the drop deadline (February 5) and prior to April 24 at 4:45 p.m.
B. Between April 24 up to 4:00 p.m. on last day of the term (May 13)
C. Anytime after the last day of the term (May 13)
There are two very broad categories of allowable reasons (extenuating circumstances) for dropping a course:
WA = Serious and compelling reasons (but reasons over which there might have been an opportunity to anticipate and drop the course prior to the drop deadline)
WB = Serious and compelling reasons that are clearly beyond your control in accordance with University Policy S09-7.

Fee Disclaimer

The CSU makes every effort to keep student costs to a minimum. Fees listed in published schedules or student accounts may need to be increased when public funding is inadequate. Therefore, CSU must reserve the right, even after initial fee payments are made, to increase or modify any listed fees, without notice, until the date when instruction for a particular semester or quarter has begun. All CSU listed fees should be regarded as estimates that are subject to change upon approval by The Board of Trustees.

You may use a printed copy from your student account to present to an employer for reimbursement or for tax reporting purposes.

A $45 late fee will be assessed depending on your late registration date. Refer to the OU calendar for specific dates. Billing statements will not be sent and students are responsible for checking their MySJSU account for the payment due date. Failure to pay fees by the due date may result in classes being dropped without notice.

International Students

If you are studying at SJSU with an F or J student visa, any courses taken through Open University will not count toward the full time enrollment required for maintaining your visa status. If you have questions, please visit the Office of International Programs and Services in Clark Hall Room 543 or call 408-924-5920.

International students who are academically disqualified must see an International Student Advisor prior to enrollment in Open University.

Payment of Tuition and Fees

Remember that your payment must be received by the due date – postmarks are not accepted. Failure to pay fees by the due date may result in the cancellation of your class(es). Review your MySJSU account for the payment due date.

 

Returned Checks and Declined Credit Cards

If you pay your fees by personal check or eCheck, please note that if your check is returned to the university unpaid for any reason, your registration is subject to cancellation of all courses for this session without prior notification. A $25 fee is charged for the first dishonored check or eCheck and $35 per check thereafter. An additional $20 administrative fee per check may also be charged. If your credit card is declined for any reason, your registration is subject to cancellation without prior notice.

Refunds

For dropping class before the deadline (see OU calendar), fees are reversed 75% minus a $20.00* processing fee. Drops thereafter will not receive a refund. Refunds are subject to application of other financial obligations owed the university.

* Sign up for Direct Deposit (eRefund) www.sjsu.edu/bursar/news/ to avoid the processing fee.
Refunds are not processed during the add/drop period.

Student Record Holds

San José State University has authority to withhold registration privileges for any student with an outstanding balance due on their account. If a Bursar hold has been placed on your records, your registration will not be processed. To resolve the debt, please call the Bursar’s Office at 408-924-1601.

Use of Social Security Number

Applicants are required to include their correct social security number in designated places on applications for admission pursuant to the authority contained in Section 41201, Title 5, California Code of Regulations, and Section 6109 of the Internal Revenue Code (26 U.S.C. 6109). The University uses the social security number to identify students and their records including identification for purposes of financial aid eligibility and disbursement and the repayment of financial aid and other debts payable to the institution.

Also, the Internal Revenue Service requires the University to file a 1098-T form for degree seeking students that include the student’s social security number. This information is used by the IRS to help determine whether a student, or a person claiming a student as a dependent, may take a credit or deduction to reduce federal income taxes.
